Connected Devices
Collect biometric data and easily share it with your clinical team through connected devices like glucometers, blood pressure cuffs and weight scales. Patients who qualify can also receive a Hero medication dispenser for tracking medication compliance. Every patient receives a personalized care plan, milestone check ins and progress reports.

24 Hour Clinical
Team Access
Providers available to work in tandem with your existing clinical team for emergency and urgent care purposes. Direct line gives access to around the clock support and intervention from your clinical team.

Covered By Medicare
All of our services are covered by Medicare. In many cases, patients pay $0 per month. A full verification is provided prior to beginning services.
